Results: Among 128 HIV-infected patients hospitalised with tuberculosis, the mean age was 38.4 years; 79.7% were men. HIV was diagnosed on admission in 28.9% of patients. The median CD4 + T-cell count was 125 (±158 SD) cells/µL. Only 47.3% of patients with a known diagnosis of HIV upon admission were on antiretroviral therapy, and only 11.1% had a tuberculin skin test in the previous year. Drug toxicity due to tuberculosis medications occurred in 11.7% of patients. Mean length of stay was 23.2 days, and 10.7% of patients were readmitted. Mortality was 5.5%. Conclusions: Hospital mortality attributable to tuberculosis in patients with HIV is low in reference hospitals in Colombia. Cases of tuberculosis in HIV-infected patients occur mainly in patients with advanced HIV, or not on antiretroviral therapy, despite a known diagnosis of HIV. Only one of every 10 patients in this cohort had active screening for latent tuberculosis, possibly reflecting missed treatment opportunities. Resultados: Se incluyeron 100 pacientes de los que el 71% eran del sexo femenino. En los antecedentes patológicos se observó que el 9, 7, 6 y 4% tenían diagnóstico de lupus, artritis, síndrome antifosfolipídico y síndrome de Sjögren, respectivamente. En los antecedentes farmacológicos los principales medicamentos reportados fueron los inmunosupresores con un 58%, entre los que destacaron los esteroides con un 33%. Las lesiones dermatológicas principales fueron maculopapulares (33%), habones (16%) y úlceras (15%). Conclusiones: Entre los diversos antecedentes patológicos existentes los principales son las enfermedades autoinmunitarias, lo que debe tenerse en cuenta para establecer el diagnóstico y una relación causal. Methods: This multicentre cohort study included patients who received any LRT before LT from Europe and Latin America (2000–2018). We excluded patients with alpha-foetoprotein (AFP) above 1,000 ng/ml. Competing risk regression analysis for HCC recurrence was conducted, estimating subdistribution hazard ratios (SHRs) and corresponding 95% CIs. Results: From 2,441 LT patients, 70.1% received LRT before LT (n = 1,711). Of these, 80.6% were within Milan, 12.0% within UCSF-DS, and 7.4% all-comers. Successful downstaging was achieved in 45.2% (CI 34.8–55.8) and 38.2% (CI 25.4–52.3) of the UCSF-DS group and all-comers, respectively. This manuscript reports the results of a single-country consensus designed to define the minimum standardized dataset to be recorded within an institutional arthroplasty registry. Methods: A national consensus was carried out among all members of the Colombian Society of Hip and Knee Surgeons using the Delphi method. Eleven questions and answers comprising every potential domain of an institutional registry of hip and knee arthroplasty were defined. According to the methodology, anonymous voting and multiple discussion rounds were performed. Three levels of agreement were defined: Strong consensus: equal to or greater than 80%, weak consensus between 70 and 79.9%, and no consensus below 70%. Results: All of the questions reached consensus level. The minimum dataset was defined to include demographic and clinical information, intraoperative and implant details, follow-up and early complications, implant survival, and functional outcome scores, as well as the validation model to assess information quality within the database. Currently, this dataset is being implemented voluntarily by the members of our national society. Discussion: A national consensus is a feasible method to build homogeneous arthroplasty registries. We recommend such an exercise since it establishes the basis to compare and add data between institutions and the joint analysis of said information in a national registry. Este estudio tiene como objetivo evaluar los desenlaces a corto y largo plazo post-trasplante renal de acuerdo a la modalidad de diálisis previa al trasplante. Métodos: estudio tipo cohorte restropsecitva. Se incluyeron pacientes mayores de 18 años que recibieron un trasplante renal durante 2005-2018. Inicalmente se realizó estadística descriptiva. Posteriormente se realizó análisis multivariado mediante un modelo de riesgos proporcionales de Cox con el objetivo de evaluar la relación entre la supervivencia del paciente y del injerto y la necesidad de diálisis previo al trasplante. Se evaluó además la tasa de filtración glomerular durante los primeros dos años del trasplante renal. Resultados: durante 2005-2018 se realizaron 925 trasplantes renales, 289 estaban en diálisis peritoneal, 439 en hemodiálisis y 197 no estaban en diálisis. La supervivencia del paciente y del injerto a 6, 12, 24, 48, 60 meses posterior al trasplante renal fue de 97.5%, 96.7%, 96.0%, 93.7%, 92.3% y 94.5%, 92.9%, 90.5%, 84.8%, 81.1% respectivamente, sin encontrar diferencia estadísticamente significativa entre los que tuvieron diálisis peritoneal vs. hemodiálisis (p=0.402, p=0.180) tampoco se encontraron diferencias significativas en el análisis multivariado. El 8.1% de los pacientes presentó rechazo agudo en el primer año post trasplante, y el 13.2% durante todo el seguimiento, sin encontrarse diferencias significativas entre los que habían tenido diálisis peritoneal vs. hemodiálisis. No se encontraron diferencias en la TFG. Conclusiones: no se encontraron diferencias estadísticamente significativas en los desenlaces duros a corto y largo plazo según la modalidad de diálisis pre-trasplante. Materials and Methods: A prospective multicentric study in 10 radiological hospitals and institutions of Latin America was performed and 818 thyroid nodules were analyzed by ultrasound and classified by using both ACR TIRADS® and Alpha Score; fine-needle aspiration biopsy was performed when needed and classified with Bethesda. The relationships between predictors were analyzed by using binary logistic regression, statistical significance was defined by a p-value of 0.05, with an error margin of 4% and 95% confidence intervals. Results: Alpha Score 2.0 establishes five types of malignant predictors: microcalcifications, irregular borders, taller-than-wide shape, predominant solid texture and hypoechogenicity; a diameter equal to or greater than 1.5 cm adds an extra point to the final score. Resulting classification divides TNs into 4 categories: benign (1.9%), low suspicion (8.7%), mild suspicion (13.6%) and high suspicion (75.7%) of malignancy probability; sensitivity of 82%, specificity of 74%, the positive predictive value of 94%, the negative predictive value of 51%, the statistical accuracy of 81%, odds ratio of 108.89 and correlation with ACR TIRADS of 0.77 and Bethesda of 0.91. Conclusions: Alpha Score 2.0 has superior diagnostic accuracy and performance compared to the previously published Alpha Score and is able to classify a benign TN in a precise, safe and accurate way, avoiding unnecessary FNABs or determining the necessity of FNAB in cases of moderate to high suspicion of malignancy. Esta condición incrementa el riesgo de recaída y mortalidad. Objetivo: Caracterizar factores clínicos y moleculares de pacientes con leucemias agudas y enfermedad mínima residual detectada por citometría de flujo en una institución de alta complejidad de la ciudad de Medellín, Colombia durante los años 2015 - 2017. Metodología: Este es un estudio descriptivo retrospectivo, que incluyó pacientes con leucemia diagnosticada por citometría de flujo. Se realizó un muestreo no probabilístico de casos consecutivos. La información recolectada fue digitada en una base de datos en Excel, y el análisis se realizó a través del programa IBM SPSS Versión 24, empleando según la naturaleza de cada variable frecuencias absolutas y relativas, promedio y desviación estándar o mediana y rangos intercuartílicos según su distribución. Resultados: Se incluyó un total de 60 pacientes con predominio del sexo masculino 63,3 % (38). El diagnóstico más frecuente fue la leucemia linfoide 78,3 % (47). Del total de pacientes incluidos, 36,6% (22) fue positivo para enfermedad mínima residual; 28,3 % recibió trasplante de médula ósea y el 10 % (6) presentó compromiso de líquido cefalorraquídeo. En la segunda citometría en pacientes con enfermedad mínima residual, 90,9 % (20) expresaba CD45+. El 31,8 % (7) de los pacientes con enfermedad mínima residual presentó recaída. Conclusión: La enfermedad mínima residual es una condición frecuente en pacientes con leucemias agudas que requiere seguimiento y constituye un factor pronóstico relevante. Esta condición incrementa el riesgo de recaída y mortalidad. Objetivo: Caracterizar factores clínicos y moleculares de pacientes con leucemias agudas y enfermedad mínima residual detectada por citometría de flujo en una institución de alta complejidad de la ciudad de Medellín, Colombia durante los años 2015 - 2017. Metodología: Este es un estudio descriptivo retrospectivo, que incluyó pacientes con leucemia diagnosticada por citometría de flujo. Se realizó un muestreo no probabilístico de casos consecutivos. La información recolectada fue digitada en una base de datos en Excel, y el análisis se realizó a través del programa IBM SPSS Versión 24, empleando según la naturaleza de cada variable frecuencias absolutas y relativas, promedio y desviación estándar o mediana y rangos intercuartílicos según su distribución. Resultados: Se incluyó un total de 60 pacientes con predominio del sexo masculino 63,3 % (38). El diagnóstico más frecuente fue la leucemia linfoide 78,3 % (47). Del total de pacientes incluidos, 36,6% (22) fue positivo para enfermedad mínima residual; 28,3 % recibió trasplante de médula ósea y el 10 % (6) presentó compromiso de líquido cefalorraquídeo. En la segunda citometría en pacientes con enfermedad mínima residual, 90,9 % (20) expresaba CD45+. El 31,8 % (7) de los pacientes con enfermedad mínima residual presentó recaída. Conclusión: La enfermedad mínima residual es una condición frecuente en pacientes con leucemias agudas que requiere seguimiento y constituye un factor pronóstico relevante. Existen unas dermatosis neutrofílicas clásicas que se han relacionado con las enfermedades autoinmunitarias desde hace mucho tiempo; sin embargo, en la bibliografía se encuentran pocos casos relacionados en pacientes con lupus eritematoso sistémico. El tratamiento de estas afecciones se basa en el manejo y control de la enfermedad subyacente con inmunosupresores, como los corticosteroides, la azatioprina o el micofenolato mofetilo, pero se reportan pocos casos en los que no se obtiene respuesta adecuada a este tratamiento, lo que hace necesaria la administración de medicamentos alternativos, como la dapsona. Caso clínico: Paciente masculino de 38 años, con antecedente de lupus eritematoso sistémico, con lesiones en la piel en las que se confirmó histológicamente el diagnóstico de dermatosis neutrofílica y al correlacionar con la clínica se concluyó que se trataba de un síndrome de Sweet-like. Recibió múltiples tratamientos inmunosupresores con poca respuesta, pero obtuvo alivio completo de las lesiones con la administración de dapsona. Conclusión: Se comunica el caso de un paciente con antecedente de lupus eritematoso sistémico, diagnóstico de dermatosis neutrofílica asociada con enfermedad autoinmunitaria y tratamiento exitoso con dapsona después de tener una respuesta no satisfactoria a múltiples tratamientos inmunosupresores. The 2-year-old patient was referred from Saint Martin to the Pediatric endocrinology for evaluation of pubic hair and breast development which started 3-month prior to presentation. On exam, the patient was noted to have abdominal prominence which prompted obtaining abdominal imaging which showed an ovarian tumor which was subsequently removed. Así mismo, se explora la influencia de otras hormonas tradicionalmente menos asociadas al acné, como el cortisol, la proopiomelanocortina, la prolactina y la hormona de crecimiento, que han probado influenciar la secreción de sebo por la unidad pilosebácea. Between 80 and 90% of cases of cardiorespiratory arrest secondary to pulmonary embolism occur between 1 and 3hours after the onset of symptoms, requiring a high degree of clinical suspicion as patient management and prognosis will depend on this. We present the case of a 72-year-old man, admitted after gastrointestinal symptoms and syncope, and who presented cardiorespiratory arrest while in the emergency room. Prior to the circulatory arrest, signs of right ventricular dysfunction and intracavitary thrombi were documented, so systemic thrombolysis was administered overall during resuscitation maneuvers. Subsequently the patient evolved successfully with no cardiac or neurological dysfunction. Objective: The purpose of the study was to analyze the imaging findings of pulmonary COVID-19 in a large pediatric series. Materials and methods: Children with SARS-CoV-2 infection confirmed by either quantitative reverse transcription-polymerase chain reaction from nasopharyngeal swabs or presence of circulating immunoglobulin M (IgM) antibodies and who underwent chest radiograph or CT or both were included in this retrospective multicenter study. Three pediatric radiologists independently reviewed radiographs and CTs to identify the presence, localization, distribution and extension of pulmonary lesions. Results: We included 140 children (71 female; median age 6.3 years, interquartile range 1.6–12.1 years) in the study. Peribronchial thickening (93%), ground-glass opacities (79%) and vascular engorgement (63%) were the most frequent findings on 131 radiographs. Ground-glass opacities (91%), vascular engorgement (84%) and peribronchial thickening (72%) were the most frequent findings on 32 CTs. Peribronchial thickening (100%), ground-glass opacities (83%) and pulmonary vascular engorgement (79%) were common radiograph findings in asymptomatic children (n=25).
