El diagnóstico inicial se hace mediante la detección de anticuerpos contra el VHC, que luego requiere una prueba confirmatoria molecular, debido a que la determinación de anticuerpos no diferencia los individuos que tienen una infección activa, de aquellos con una infección resuelta y sin viremia. El objetivo del tratamiento de la infección crónica por VHC es curar la enfermedad, lo que se determina cuando se logra una respuesta viral sostenida; es decir, cuando no se detecta carga viral 12 semanas o más después de terminada la terapia. Para un tratamiento óptimo de la infección y alcanzar la curación, se recomienda evaluar previamente de manera no invasiva el estadio de fibrosis, y garantizar la adherencia durante todo el tiempo de tratamiento. En la presente revisión se incluyen las recomendaciones para el tratamiento de la infección crónica por VHC, según el genotipo viral, basadas en los medicamentos disponibles en nuestro medio y en los que próximamente estarán llegando al país. This study aimed to evaluate the independent relationship between chest imaging and mortality and examine the inter-rater variability of interpretations of chest radiographs (CXR) in pediatric ARDS (PARDS). Methods: Prospective, international observational study in children meeting Pediatric Acute Lung Injury Consensus Conference (PALICC) criteria for PARDS, which requires new infiltrate(s) consistent with pulmonary parenchymal disease, without mandating bilateral infiltrates. Mortality analysis focused on the entire cohort, whereas inter-observer variability used a subset of patients with blinded, simultaneous interpretation of CXRs by intensivists and radiologists. Results: Bilateral infiltrates and four quadrants of alveolar consolidation were associated with mortality on a univariable basis, using CXRs from 708 patients with PARDS. For patients on either invasive (IMV) or non-invasive ventilation (NIV) with PaO2/FiO2 (PF) ratios (or SpO2/FiO2 (SF) ratio equivalent) > 100, neither bilateral infiltrates (OR 1.3 (95% CI 0.68, 2.5), p = 0.43), nor 4 quadrants of alveolar consolidation (OR 1.6 (0.85, 3), p = 0.14) were associated with mortality. For patients with PF ≤ 100, bilateral infiltrates (OR 3.6 (1.4, 9.4), p = 0.01) and four quadrants of consolidation (OR 2.0 (1.14, 3.5), p = 0.02) were associated with higher mortality. A subset of 702 CXRs from 233 patients had simultaneous interpretations. Interobserver agreement for bilateral infiltrates and quadrants was “slight” (kappa 0.31 and 0.33). Subgroup analysis showed agreement did not differ when stratified by PARDS severity but was slightly higher for children with chronic respiratory support (kappa 0.62), NIV at PARDS diagnosis (kappa 0.53), age > 10 years (kappa 0.43) and fluid balance > 40 ml/kg (kappa 0.48). Conclusion: Bilateral infiltrates and quadrants of alveolar consolidation are associated with mortality only for those with PF ratio ≤ 100, although there is high- inter-rater variability in these chest-x ray parameters. Se ha calculado que en los últimos 200 años ha sido la responsable de la muerte de más de 1.000 millones de personas (1). La TB sigue siendo una de las enfermedades infecciosas más importante en esta época, y es el fiel reflejo de las enormes diferencias económicas y sociales que siguen existiendo en la población mundial. Objetivo: describir las características demográficas y clínicas de los pacientes menores de 15 años con TB en el período 2011 - 2018 en un Hospital de cuarto nivel de la ciudad de Medellín - Colombia. Metodología: estudio observacional descriptivo de corte transversal. Incluyó pacientes menores de 15 años que asistieron al Hospital Universitario San Vicente Fundación (HUSVF) a quienes se les diagnosticó TB pulmonar y/o extrapulmonar, en el período comprendido entre enero de 2011 a diciembre de 2018. Resultados: 184 historias de pacientes con TB cumplieron los criterios de inclusión. Setenta y un pacientes (38.6 %) correspondían a TB pulmonar, 59 (32 %) a otras TB intratorácicas y 54 (29.3 %) a TB extrapulmonar. Las formas de TB más frecuentes fueron: ganglionar, del sistema nervioso central (SNC) y diseminada. De los niños menores de 5 años, el 50 % presentaron algún grado de desnutrición. La confirmación microbiológica por cultivo o baciloscopia (BK) estuvo presente en 91 pacientes (49.4 %) y por Reacción en Cadena de la Polimerasa (PCR) en 72 (39.1 %). Conclusiones: la TB infantil requiere un alto índice de sospecha. La suma de criterios es muy importante para su diagnóstico dada la fisiopatología de la enfermedad, sin embargo la búsqueda exhaustiva de la micobacteria es una estrategia útil para un tratamiento efectivo. We describe the clinical scenario of a patient with recurrence of HCC five years after LT, who received lenvatinib as first-line systemic therapy to introduce systemic treatment options in this clinical setting. In this opinion review, we detail first and second-line systemic treatment options, focusing on those feasible for patients with recurrent HCC after LT. Several trials have evaluated new drugs to treat HCC patients in first and second-line therapy, but patients with recurrent HCC after LT have been excluded from these trials. Consequently, most of the evidence comes from observational retrospective studies. Whether tyrosine kinase inhibitors will remain the primary therapeutic approach in these patients, due to a relative contraindication for immunotherapy, may be clarified in the near future. Ondansetron was the only intervention that reduced the need for intravenous rehydration and the number of vomiting episodes. When considering side effects, dimenhydrinate was the only intervention that was worse than placebo. Limitations: Most treatment comparisons had low- or very low-quality evidence, because of risk of biases and imprecise estimates. Conclusions: Ondansetron is the only intervention that revealed an effect on the cessation of vomiting, on preventing hospitalizations, and in reducing the need for intravenous rehydration. They can be remarkably difficult to distinguish from epileptic seizures: both may involve alterations in mental status and behavior, sensory or perceptual disturbances, as well as simple or complex motor patterns. Pictorial expressions of PNES have always attracted attention from the lay public and medical practitioners alike, by showing the rich variety of semiologies seen in the condition and its differences with epileptic seizures. In this article, we present two new contemporary artistic descriptions by the Canadian artist Iris Hauser about PNES. The depictions highlight key signs seen in PNES such as “eyes closed during events”, “the hyperextension of the back and limbs”, and the “claw-like finger posturing”. We believe that the drawings can provide a valuable educational component in the initial assessment of patients suspected of having PNES as a training tool to identify and refer patients for more advanced testing. Objetivo: Determinar la prevalencia y caracterizar los errores de medicación en el servicio de pediatría reportados en el Sistema de notificación y gestión de riesgo clínico de un hospital universitario entre el 2017 y 2018. Metodología: Estudio de corte transversal. Se cuantificó la proporción de errores de medicación en el servicio de pediatría a partir de un informe del Sistema de notificación de riesgo clínico entre el año 2017 y 2018. Los errores de medicación se clasificaron por proceso de ocurrencia, tipo de error, subgrupo y gravedad. Para el análisis se utilizó estadística descriptiva. Resultados: Se generaron 669 reportes de eventos adversos, 376 (56,20%) estaban relacionados con medicamentos. La tasa calculada de errores fue 7,71 por cada 1000 paciente-días. La mayoría de los errores de medicación se clasificaron como Error sin daño (categoría B y C), 176 ambas subcategorías (352 total) para un 93,62% del total de errores. El proceso de prescripción reportó la mayoría de los errores 59,84%. Conclusiones: La mayoría de los errores de medicación reportados se relacionaron con la prescripción, clasificándose principalmente como errores sin daño. In this case report, we discuss a 70-year-old woman who presented with a 3-month history of a growing painless forehead lump and confusion, for which she had an emergent simple head computed tomography scan. The images revealed a well-defined mass eroding the frontal bone with multiple lytic lesions that were also found along with long bones radiography. The mass biopsy showed a monomorphic plasmatic cell infiltrate, bone marrow studies confirmed the diagnosis of a light chain secreting multiple myeloma. Extramedullary plasmacytoma is a very unusual first presentation form of multiple myeloma and represents a clinical and radiological challenge. A systematic approach of lytic bone lesions along with the differential diagnosis of head masses are skills the clinician should develop to promptly recognize this condition considering further complications of delayed treatment. In this case, the histopathological confirmation allowed the patient to avoid neurosurgery and the early start of systemic chemotherapeutic treatment. Se presenta el caso de un paciente atendido por trauma de alta cinemática en un accidente de tránsito como motociclista, con lesiones predominantes en el cráneo y la cara, en quien se documentó embolismo aéreo venoso como causa insospechada de hipoxemia profunda persistente. No se logra identificar su origen y se considera iatrogénico. El paciente es manejado exitosamente de forma conservadora en la unidad de cuidados intensivos. Se presenta el caso de un paciente atendido por trauma de alta cinemática en un accidente de tránsito como motociclista, con lesiones predominantes en el cráneo y la cara, en quien se documentó embolismo aéreo venoso como causa insospechada de hipoxemia profunda persistente. No se logra identificar su origen y se considera iatrogénico. El paciente es manejado exitosamente de forma conservadora en la unidad de cuidados intensivos. Se discutirá el caso reportado a la luz de la revisión de la literatura pertinente. Models were externally validated in a cohort of intubated pediatric acute respiratory distress syndrome patients from the Children's Hospital of Philadelphia. Setting: The derivation cohort represented 100 centers worldwide; the validation cohort was from Children's Hospital of Philadelphia. Patients: There were 624 and 640 subjects in the derivation and validation cohorts, respectively. Interventions: None. Measurements and main results: The model for the full cohort included immunocompromised status, Pediatric Logistic Organ Dysfunction 2 score, day 0 vasopressor-inotrope score and fluid balance, and PaO2/FIO2 6 hours after pediatric acute respiratory distress syndrome onset. This model had good discrimination (area under the receiver operating characteristic curve 0.82), calibration, and internal validation. Models excluding neurologic deaths, for intubated subjects, and for intubated subjects excluding neurologic deaths also demonstrated good discrimination (all area under the receiver operating characteristic curve ≥ 0.84) and calibration. In the validation cohort, models for intubated pediatric acute respiratory distress syndrome (including and excluding neurologic deaths) had excellent discrimination (both area under the receiver operating characteristic curve ≥ 0.85), but poor calibration. After revision, the model for all intubated subjects remained miscalibrated, whereas the model excluding neurologic deaths showed perfect calibration. Mortality models also stratified ventilator-free days at 28 days in both derivation and validation cohorts. Conclusions: We describe predictive models for mortality in pediatric acute respiratory distress syndrome using readily available variables from day 0 of pediatric acute respiratory distress syndrome which outperform severity of illness scores and which demonstrate utility for composite outcomes such as ventilator-free days. Models can assist with risk stratification for clinical trials. Es un virus altamente contagioso; hasta el momento, la enfermedad por coronavirus (COVID-19) presenta una mortalidad global alrededor del 6 %, que es mayor en pacientes con comorbilidades. La enfermedad inflamatoria intestinal (EII) es una patología discapacitante, con daño tisular y alteración en la respuesta inmunológica, lo cual, asociado a los medicamentos inmunosupresores que se utilizan frecuentemente para su tratamiento, pone al paciente en riesgo de desarrollar infecciones y complicaciones. En esta revisión consideramos la interacción del virus SARS-CoV-2 con el tracto gastrointestinal y los potenciales mecanismos por los cuales un paciente con EII podría tener un riesgo incrementado de la infección por COVID-19. Adicionalmente, y a pesar de que no hay estudios clínicos publicados en pacientes con EII y COVID-19, damos recomendaciones basadas en opinión de expertos sobre el cuidado de pacientes con EII, con énfasis en su tratamiento y la realización segura de procedimientos endoscópicos, tanto para el paciente como para el personal de salud. Se analizaron datos de producto dosis longitud (DLP) para TC de cráneo, tórax, abdomen y TC de tórax de alta resolución (TACAR); y producto dosis área (DAP) para radiografía de tórax, abdomen, huesos y fluoroscopia por grupos etarios. Resultados: Se obtuvieron los datos de 780 pacientes: 360 radiografías, 100 de fluoroscopia y 320 tomografías. Se encontró que los niveles de referencia de dosis de radiación usados en el hospital son bajos, comparados con guías europeas de 2018 de niveles de referencia. Se describen DRL para cada estudio y grupo etario. Conclusión: Se demostró que en el hospital donde hizo el estudio los niveles de referencia de radiación en la población pediátrica son bajos. El presente trabajo puede servir como referente nacional. We did not find significant differences between GT and placebo for diarrhoea duration (mean difference (MD)=−15.85 hours; 95% CI −42.24 to 14.82, I2=92%; three studies), stool frequency at day 2 (MD=0.11 stools/day; 95% CI −0.39 to 0.62: I2=26%; two studies), diarrhoea at day 3 (risk ratio [RR]=0.46; 95% CI 0.06 to 3.47: I2=73%; two studies), vomiting (RR=1.31; 95% CI 0.95 to 1.80: I2=0%; two studies) or adverse events (RR=0.86; 95% CI 0.27 to 2.66: I2=0%; two studies). Most common adverse events included abdominal pain and nausea. Conclusion: The effect of GT was no different to placebo for mean diarrhoea duration (low certainty on the evidence) and stool frequency at day 2 (high certainty) and for the presence of diarrhoea at day 3 (very low certainty) of vomiting (moderate certainty) and of adverse events (low certainty).
