Objective:To assess the efficacy of opioids in adults with chronic low back pain (CLBP). Summary of Background Data. Opioids for CLBP has increased dramatically. However, the benefits and risks remain unclear. Methods: We updated a 2007 Cochrane Review through October 2012 of randomized controlled trials from multiple databases. Use of noninjectable opioids in CLBP for at least 4 weeks was compared with placebo or other treatments; comparisons with different opioids were excluded. Outcomes included pain and function using standardized mean difference (SMD) or risk ratios with 95% confidence intervals (CIs), and absolute risk difference with 95% CI for adverse effects. Study quality was evaluated using Grading of Recommendations Assessment, Development, and Evaluation criteria. Results: Fifteen trials (5540 participants), including twelve new, met the criteria. Tramadol was better than placebo for pain (SMD, −0.55; 95% CI, −0.66 to −0.44) and function (SMD, −0.18; 95% CI, −0.29 to −0.07). Compared with placebo, transdermal buprenorphine decreased pain (SMD, −2.47; 95% CI, −2.69 to −2.25), but not function (SMD, −0.14; 95% CI, −0.53 to 0.25). Strong opioids (morphine, hydromorphone, oxycodone, oxymorphone, and tapentadol), were better than placebo for pain (SMD, −0.43; 95% CI, −0.52 to −0.33) and function (SMD, −0.26; 95% CI, −0.37 to −0.15). One trial demonstrated little difference with tramadol compared with celecoxib for pain relief. Two trials (272 participants) found no difference between opioids and antidepressants for pain or function. Reviewed trials had low to moderate quality, high drop-out rates, short duration, and limited interpretability of functional improvement. No serious adverse effects, risks (addiction or overdose), or complications (sleep apnea, opioid-induced hyperalgesia, hypogonadism) were reported. Conclusion. There is evidence of short-term efficacy (moderate for pain and small for function) of opioids to treat CLBP compared with placebo. The effectiveness and safety of long- term opioid therapy for treatment of CLBP remains unproven. Clínicamente se presenta como una masa asintomática, subcutánea, unilateral, de tamaño variable con múltiples diagnósticos diferenciales, por lo cual, el examen histopatógico es necesario para llegar al diagnóstico. Los fibroadenomas vulvares son tumores fibroepiteliales bifásicos conformados por un componente glandular epitelial y otro estromal, similares a los hallados en mama. La histogénesis de este tipo de tumores es incierta. El tratamiento de este tipo de lesión es la resección, siendo su pronóstico excelente. Se presenta un caso de una paciente de 42 años, sin enfermedades de base, la cual presenta una masa asintomática en zona vulvar. En los pacientes trasplantados de riñón el estrés quirúrgico, los medicamentos inmunosupresores y las estancias prolongadas en el hospital puede ser factores que favorezcan la hiperglucemia. Sin embargo, no se ha confirmado si la hiperglucemia durante el postrasplante inmediato se asocia con mayor riesgo de rechazo. Población y métodos: Estudio de cohorte retrospectivo en pacientes mayores de 15 años que recibieron un trasplante renal y que contaron con mediciones de glucemia en las primeras 48 h postrasplante. Se evaluó la presencia de hiperglucemia definida de 3 maneras diferentes (como un valor aislado, como la media de los valores y como un valor ponderado en el tiempo) en pacientes sometidos a un trasplante renal y su relación con el riesgo de rechazo agudo y el tiempo de estancia hospitalaria. Resultados: Si bien un número elevado de pacientes (91%) presentaron alguna forma de hiperglucemia durante las primeras 48 h postrasplante, independientemente de cómo esta fue definida, no hubo un mayor riesgo de rechazo (OR: 0,35; IC 95%: 0,11-1,08) o diferencia en el tiempo de estancia hospitalaria (13,2 vs. 8,9 días, p = 0,958) entre los pacientes que la presentaron y los que no. Conclusiones: Es frecuente encontrar algún tipo de hiperglucemia durante las primeras 48 h después del trasplante renal; sin embargo, su presencia no acarrea mayor riesgo de rechazo del trasplante o mayor tiempo de estancia hospitalaria cuando se compara con pacientes que no la presentaron previamente. In Latin America, which is considered an endemic area for this mycosis, there have been no published reports regarding the incidence, clinical presentation, morbidity, and mortality of histoplasmosis in renal transplant patients. The objective of this study was to describe cases of histoplasmosis in renal transplant patients treated at the Pablo Tobon Uribe Hospital (Medellin, Colombia) between 2006 and 2013. Methods: This is a descriptive, retrospective study. Results: The incidence of histoplasmosis in our renal transplant population was 1.1%. The ages of the 9 patients (4 men and 5 women) ranged between 27 and 59 years. In 2 of these patients, histoplasmosis appeared during the first year after transplantation. At the time of transplantation, 66% of patients received induction with alemtuzumab; 88% had a prior rejection episode and required increased immunosuppressive medication; 88% had renal graft dysfunction with creatinine levels >1.5 mg/dL; and the primary clinical presentation was disseminated histoplasmosis followed by the pulmonary form of the disease. Diagnoses were performed by histology in 6 patients, blood culture in 2 patients, and antigenuria in 1 patient. Three patients required treatment with amphotericin B for the severity of their infection, and 2 of these patients died before receiving the cumulative dose of amphotericin B. The 7 remaining patients received itraconazole for 12 months and had a successful treatment response. Regarding complications, 2 patients had hemophagocytic syndrome. At the 1-year follow-up appointment, renal function remained stable in all patients, and no patients had acute rejection or required renal replacement therapy. Thus, the overall mortality rate observed was 22.2%. Conclusions: In this series, histoplasmosis in renal transplant patients presented as an aggressive opportunistic infection with a higher incidence than that previously reported in the literature. The following risk factors have been associated with histoplasmosis: renal graft dysfunction, previous acute rejection, immunosuppression with tacrolimus-mycophenolate, and induction with alemtuzumab. The clinical presentation of histoplasmosis was nonspecific, which complicated disease diagnosis, and the treatment regimens were highly toxic and associated with significant morbidity and mortality rates. Resumen : Background: Histoplasmosis is an uncommon systemic fungal infection, but it is potentially fatal in immunosuppressed populations. In Latin America, which is considered an endemic area for this mycosis, there have been no published reports regarding the incidence, clinical presentation, morbidity, and mortality of histoplasmosis in renal transplant patients. The objective of this study was to describe cases of histoplasmosis in renal transplant patients treated at the Pablo Tobon Uribe Hospital (Medellin, Colombia) between 2006 and 2013. Methods: This is a descriptive, retrospective study. Results: The incidence of histoplasmosis in our renal transplant population was 1.1%. Re-analysis of these data was performed to assess infections. Heterogeneity was also explored. A subgroup analysis was performed according to elective/emergency surgery status. Results: Data from 72 RCTs including 8218 patients were collected (4116 patients in the laparoscopic group and 4102 patients in the open group). For superficial SSI, the pooled RD was -4.4% (95% CI: -5.4% to -3.3%), which indicated a lower risk in the laparoscopic group. For organ/space SSI, the pooled RD was 0.5% (95% CI: -0.1% to 1%), which indicated similar rates between the groups. Changes in SSI frequency had occurred over time. Conclusion: Laparoscopic surgery significantly decreases the risk of superficial SSI but does not affect the risk of organ/space SSI. Experience with technique improves outcomes. Objetivos: evaluar la función tiroidea en pacientes eutiroideos después de la exposición a medios de contraste iodado. Metodología: evaluamos 100 pacientes ambulatorios programados para imágenes con medios de contraste iodado en un centro hospitalario. Previo a su realización y tres meses después, se obtuvieron muestras para hormona tiroestimulante (TSH), tiroxina libre (T4L), anticuerpos anti-peroxidasa (anti-TPO) y tiroglobulina (Tg). Los datos obtenidos fueron procesados en SPSS, se obtuvieron tablas de frecuencia y de contingencia; se estableció significancia estadística a través de pruebas de t-student para muestras relacionadas y test exacto de Fisher. Resultados: evaluamos 100 pacientes, 67% mujeres. Edad promedio: 53,7 años, 41% tuvo 60 o más años. A 66 se les realizó tomografía axial computarizada (TAC) contrastada, a 27 angiografía y a 7, urografía excretora. El 2% tuvo TSH menor a 0.5 uUI/ml y 17% valores mayores o iguales a 4.5 uUI/ml. 99 (99%) tuvieron T4L normal y 1 (1%) un valor menor de 0.8 ng/dl. 11% presentó anti TPO igual o mayor a 35 UI/ml. 6 (6%) con valores de Tg mayores o iguales a 79 ng/ml. Seguimos 89 pacientes; 11 (12,3%) desarrollaron hipotiroidismo subclínico, no encontramos hipertiroidismo. Observamos una diferencia estadísticamente significativa entre los valores previos y posteriores de la TSH. Objetivos: evaluar la función tiroidea en pacientes eutiroideos después de la exposición a medios de contraste iodado. Metodología: evaluamos 100 pacientes ambulatorios programados para imágenes con medios de contraste iodado en un centro hospitalario. Previo a su realización y tres meses después, se obtuvieron muestras para hormona tiroestimulante (TSH), tiroxina libre (T4L), anticuerpos anti-peroxidasa (anti-TPO) y tiroglobulina (Tg). Los datos obtenidos fueron procesados en SPSS, se obtuvieron tablas de frecuencia y de contingencia; se estableció significancia estadística a través de pruebas de t-student para muestras relacionadas y test exacto de Fisher. Resultados: evaluamos 100 pacientes, 67% mujeres. Edad promedio: 53,7 años, 41% tuvo 60 o más años. A 66 se les realizó tomografía axial computarizada (TAC) contrastada, a 27 angiografía y a 7, urografía excretora. El 2% tuvo TSH menor a 0.5 uUI/ml y 17% valores mayores o iguales a 4.5 uUI/ml. 99 (99%) tuvieron T4L normal y 1 (1%) un valor menor de 0.8 ng/dl. 11% presentó anti TPO igual o mayor a 35 UI/ml. 6 (6%) con valores de Tg mayores o iguales a 79 ng/ml. Seguimos 89 pacientes; 11 (12,3%) desarrollaron hipotiroidismo subclínico, no encontramos hipertiroidismo. Observamos una diferencia estadísticamente significativa entre los valores previos y posteriores de la TSH. Conclusión: luego de la exposición a medios de contraste iodado es frecuente observar hipotiroidismo subclínico (21,3%), especialmente en mayores de 60 años. Defining aggressiveness is of crucial importance for improving the management of patients by enhancing prognostic predictions and effectiveness of treatment. New drugs, such as temozolomide, have potential use in the management of these patients; anti-VEGF therapy, mTOR and tyrosine kinase inhibitors are also potentially useful in managing selected patients. Different measures have been used to avoid this damage, but the evidence is controversial. New investigations are required to clarify it. We investigated the efficacy and safety of sodium bicarbonate solution compared with sodium chloride solution to prevent contrast induced nephropathy in patients with or at risk of renal dysfunction. Methods: A prospective, single-center, randomized clinical trial conducted from May 1, 2007 to February 8, 2008. Inpatients in a tertiary center, scheduled to undergo a procedure with the nonionic radiographic contrast agent iohexol. There were 220 patients with serum creatinine levels of at least 1.2 mg/dL (106.1 µmol/L) and/or type 2 diabetics, who were randomized to receive an infusion of sodium chloride (n = 113) or sodium bicarbonate (n = 107) before and after contrast dye administration. The intervention were "A" group received 1 ml/kg/hour of normal saline solution, starting 12 hours before and continuing 12 hours after iohexol contrast. "B" group received 3 ml/kg of sodium bicarbonate solution (150 mEq/L) one hour prior to procedure and then drip rate was decreased to 1 ml/kg/hour until 6 hours post procedure. Our main outcome measure was change in serum creatinine. Results: The mean creatinine value after the procedure was 1.26 mg/dL in the saline group and 1.22 mg/dL in the bicarbonate group (mean difference: 0.036; CI 95%: -0.16 to 0.23, p = 0.865). The diagnosis of contrast-induced nephropathy, defined by increase in serum creatinine on 25% or more within 2 days after administration of radiographic contrast, was done in twelve patients (12%) in the bicarbonate group and eighth patients (7.1%) in the saline group (RR: 1.68, CI 95%: 0.72 to 3.94). Conclusion: Our investigation showed that there were no differences between normal saline solution (extended infusion) vs. bicarbonate solution for nephroprotection.
