The clinical presentation depends on the degree of involvement and can range from chest pain in 85-96% of cases, acute coronary syndrome with ST-elevation (STEACS) in 20-50%, ventricular arrhythmias in 3-5%, cardiogenic shock in 2%, sudden death or cardiac tamponade. Some predisposing factors include female gender, pregnancy, age. Coronary angiography is the method of choice for diagnosis and classification. Regarding its treatment, conservative management is preferred in stable patients. The case of a patient with STEACS and cardiac arrest secondary to spontaneous coronary dissection while performing a physical activity is presented.
