Pathology of GH-producing pituitary adenomas and GH cell hyperplasia of the pituitary / Luis Vicente Syro Moreno
Título : Pathology of GH-producing pituitary adenomas and GH cell hyperplasia of the pituitary Tipo de documento : documento electrónico Autores : Luis Vicente Syro Moreno, Fecha de publicación : 2017 Títulos uniformes : Pituitary Idioma : Inglés (eng) Palabras clave : Acromegaly classification diagnosis immunohistochemistry pathology pituitary neoplasms Resumen : Introduction: Histologic, immunohistochemical and electron microscopic studies have provided conclusive evidence that a marked diversity exists between tumors which secrete growth hormone (GH) in excess. GH cell hyperplasia can also be associated with acromegaly in patients with extrapituitary GH—releasing hormone secreting tumors or in familial pituitary tumor syndromes. Materials and methods: A literature search was performed for information regarding pathology, GH-producing tumors and acromegaly. Results: This review summarizes the current knowledge on the morphology of GH-producing and silent GH adenomas, as well as GH hyperplasia of the pituitary. Conclusion: The importance of morphologic classification and identification of different subgroups of patients with GH-producing adenomas and their impact on clinical management is discussed. Initially considered as sporadic tumours, some of them are associated with familial syndromes such as familial isolated pituitary adenoma, multiple endocrine neoplasia type 1 and type 4, X-linked acrogigantism syndrome, Carney complex, pheochromocytoma/paraganglioma–pituitary adenoma, pituitary blastoma and McCune–Albright syndrome. They represent a group of diseases with different genetic background and variable phenotype. Here, we summarize the clinicopathological features of pituitary adenomas associated with these familial syndromes. Mención de responsabilidad : Luis V. Título : Current status on histological classification in Cushing’s disease Tipo de documento : documento electrónico Autores : Luis Vicente Syro Moreno, Fecha de publicación : 2015 Títulos uniformes : Pituitary Idioma : Inglés (eng) Palabras clave : Classification cushing’s disease diagnosis pathology pituitary tumor Resumen : Introduction: Managing Cushing’s disease remains a challenge. Surgery is the first option of treatment and it offers a high success rate. Even in cases where biochemical remission is not achieved, it is crucial to obtain surgical tissue for morphological diagnosis because the therapeutic approach can be modified according to the findings. Materials and Methods: A literature search was performed using PubMed for information regarding pathology and Cushing’s disease. Results: The histopathological features found in the pituitary gland of patients with Cushing’s disease are presented. Conclusion: Different subtypes of ACTH-producing pituitary tumors are recognized and characterized. The significance of finding a normal pituitary gland with or without Crooke’s changes is also discussed. Título : Improving differential diagnosis of pituitary adenomas Tipo de documento : documento electrónico Autores : Carlos Esteban Builes Montaño, ; Luis Vicente Syro Moreno, Fecha de publicación : 2014 Títulos uniformes : Expert Review of Endocrinology and Metabolism Idioma : Inglés (eng) Palabras clave : Classification diagnosis familial syndrome genetics pathology pituitary adenoma Resumen : Pituitary adenomas are common tumors arising in adenohypophysial cells or their precursors. For improving control of the disease an early diagnosis is important. Initially considered sporadic tumors, some of them are associated with familial syndromes and their recognition and classification is also required. Morphologically, pituitary adenomas represent a heterogeneous group of tumors with several subtypes and different clinical behavior thus a precise pathological diagnosis is crucial. The simple diagnosis of pituitary adenoma is not satisfactory and the correct classification of histological subtypes may predict aggressiveness in the majority of cases. Although considered not malignant, some of them are clinically aggressive and their recognition remains a challenge. In this paper we present the recent advances in the event of improving early recognition and differential diagnosis of pituitary tumors. Título : Intra- and interobserver agreement on the Oestern and Tscherne classification of soft tissue injury in periarticular lower - limb closed fractures Otros títulos : Condordancia intra e interobservador de la clasificación de Oestern y Tscherne para lesiones de tejidos blandos en fracturas cerradas periarticulares del miembro inferior Tipo de documento : documento electrónico Autores : Mauricio Estrada Castrillón, ; Carlos Oliver Valderrama Molina, Fecha de publicación : 2014 Títulos uniformes : Colombia Médica Idioma : Inglés (eng) Palabras clave : Soft tissue injuries classification tibial fractures Reliability Resumen : Background: The soft tissues injury in periarticular fractures of the lower extremities determines the proper time to perform bone fixation. Objetive: The aim of this study was to determine the intra and interobserver agreement in the Tscherne classification. Methods: This is a descriptive, prospective study for patients admitted to the Pablo Tobón Uribe Hospital (PTUH) with tibial plateau or tibial pilon fractures. We performed a standardize evaluation using video photography at the time of admission and 24, 48, and 72 h after admission. Fifteen five reviewers who had various levels of training produced a total of 1,200 observations. The intra- and interobserver agreement was assessed using a weighted kappa for multiple raters and more than two categories. Results: Twenty patients were admitted with tibial plateau and tibial pilon fractures. The intraobserver agreement for all 15 raters was kappa 0.81 (95% CI 0.79-0.83), and the interobserver agreement for all 15 raters was kappa 0.65 (95% CI 0.55-0.73). The interobserver agreement at 24 h was kappa 0.67 (95% CI 0.46-0.86). Conclusions:Classifying the severity of soft tissue injury is critical in planning the surgical management of fractures of the lower extremities. Based on our results, we can reasonably argue that the Tscherne classification produced an adequate level of agreement and could be used to standardize and to guide the treatment, and to conduct research studies.
