Según la gravedad de la enfermedad, se administró ITZ en dosis de 100 mg / día (89 casos) o de 200 mg / día (21 casos). La duración media de la terapia fue de 18 meses en ASP, 13 en CMB, 7 en PCM y 5 en SPT. La ITZ fue bien tolerada y solo el 6,3% de los pacientes se quejaron de intolerancia gástrica leve. Las pruebas de función hematológica y hepática se mantuvieron dentro de los límites de la normalidad en todos los pacientes, incluso aquellos que presentaron disfunción hepática antes de la cirugía, mejoraron durante el tratamiento con ITZ. All patients were clinically and bacteriologically cured. No significant adverse reactions were observed. The results suggest that administration of a single daily dose of 20 mg/kg amikacin should be considered practical and safe in children. Further studies are needed. The various types of conidia observed in the sections correlated well with previous light-microscopic descriptions. These types were intercalary or apical conidia, depending on their location along the originating hyphae. As in previous studies they were characterized as arthroconidia, aleuriospores and sessile or pedunculate pyriform conidia. The sporogenous cells were clearly distinguished from hyphal cells by the thickness and appearance of their cell walls. Copious fibrillar material (glycocalyx) detected at the cell surface was stained with ruthenium red during the fixation process. Typical subcellular organelles (nucleus, nucleolus, mitochondria, ribosomes, etc) were found in most of the sections. It was concluded that the spores produced by the mycelial phase of P. brasiliensis possess all attributes of viable and physiologically competent eukaryotic cells. La edad media fue de 66,5 años (37-87); 16 eran mujeres (88,8%) y 2 varones (11,1%). A 10 pacientes mujeres se les había realizado histerectomía vaginal (9) o el diagnóstico de histerocele asociado (1). La técnica de Orr-Loygue se utilizó en 8 pacientes. (44,4%), técnica de Notaras en 5 pacientes (27,7%) y técnica de Ripsteln en 5 pacientes (27,7%), 3 de los pacientes S presentaron tres complicaciones tempranas en la técnica de Ripsteln (defecación difícil, distensión abdominal, perforación rectal). No se presentaron complicaciones con las otras dos técnicas. Una patente murió en el postoperatorio inmediato (5,5%). En total, los 17 pacientes restantes tuvieron un seguimiento clínico: excelente en 11 (S.3 años) y bueno en 4 pacientes (7.2 meses); dos pacientes no tuvieron seguimiento. The majority of the patients (45) had the chronic adult form of the disease while the remaining exhibited the juvenile form. All of the patients received itraconazole, 100 mg day-1, with the exception of two for whom the clinical condition necessitated a larger dose. The mean duration of therapy was 6 months, with a range of 3–24 months. Clinically, the skin and mucous membrane lesions as well as abdominal pain, auscultatory alterations and dysphonia improved markedly during treatment with a concomitant increase in weight. However, other symptoms (cough, expectoration and dyspnoea) persisted as sequelae in some cases. As expected, adrenal insufficiency also persisted. The radiological lesions showed a gradual decrease of both scattered and confluent infiltrates, present in 67% of patients before, and in 13% at the end of treatment. On the other hand, fibrosis became more evident at the termination of therapy, increasing from 62% of patients at the beginning of therapy to 81% at the end. The mycological tests (direct examination and cultures) became negative during the first month of treatment in 42 patients (87%). A decline in specific antibody titres was observed in 72% of patients by the end of treatment. Evaluation of therapy by means of a scoring system indicated complete resolution of the disease in one patient (2%), marked improvement in 42 (89%) and minor improvement in four (8·5%); none of the patients showed a deterioration of their disease during therapy. Of fifteen patients who were followed up for 12 months post-therapy none showed clinical relapse during this period. Resumen : Forty-seven patients with active paracoccidioidomycosis were treated with itraconazole. The majority of the patients (45) had the chronic adult form of the disease while the remaining exhibited the juvenile form. All of the patients received itraconazole, 100 mg day-1, with the exception of two for whom the clinical condition necessitated a larger dose. The mean duration of therapy was 6 months, with a range of 3–24 months. Clinically, the skin and mucous membrane lesions as well as abdominal pain, auscultatory alterations and dysphonia improved markedly during treatment with a concomitant increase in weight. However, other symptoms (cough, expectoration and dyspnoea) persisted as sequelae in some cases. As expected, adrenal insufficiency also persisted. The radiological lesions showed a gradual decrease of both scattered and confluent infiltrates, present in 67% of patients before, and in 13% at the end of treatment. On the other hand, fibrosis became more evident at the termination of therapy, increasing from 62% of patients at the beginning of therapy to 81% at the end. The mycological tests (direct examination and cultures) became negative during the first month of treatment in 42 patients (87%). A decline in specific antibody titres was observed in 72% of patients by the end of treatment. Evaluation of therapy by means of a scoring system indicated complete resolution of the disease in one patient (2%), marked improvement in 42 (89%) and minor improvement in four (8·5%); none of the patients showed a deterioration of their disease during therapy. Of fifteen patients who were followed up for 12 months post-therapy none showed clinical relapse during this period. Four isolates of P. brasiliensis, including recently isolated from patients or experimental animals, caused chronic progressive disease. Two isolates with a history of subculturing showed attenuation by causing resolving but chronic disease. An attenuated isolate became avirulent subsequent to 15 more years of subculturing. These findings suggest that virulence of P. brasiliensis can be attenuated or lost subsequent to cycles of subculturing over long periods. Our data suggest that the use of fresh P. brasiliensis isolates may be needed to provide reproducible virulence for experimental systems. Streptococcus pneumoniae, identified in 26 cases (22%), was the second most common pathogen. All H. influenzae and S. pneumoniae strains were susceptible to ampicillin and penicillin, respectively. We conclude that amoxicillin remains the drug of choice for treatment of acute otitis media in our country. Bacteria were isolated in 82 patients (7.4%). Haemophilus influenzae, the most common isolate, was present in 40 cases (36%); 32 were nontypable strains and 8 were type b. For Enterobacteriaceae, 0.5 μg/ml inhibited 100% of the indole-positive Proteus and Salmonella spp.; 1 μg/ml inhibited 100% of Shigella and Citrobacter, 2 μg/ml inhibited 100% of Enterobacter spp., E. coli, Proteus mirabilis, and Serratia marcescens isolates tested. The MIC90 for Klebsiella spp. and Pseudomonas spp. was 4 μg/ml. The MIC90 was 4 μg/ml for S. aureus and 2 μg/ml for S. epidemidis and S. saprophyticus. The MIC90 of Streptococcus spp. were ⩽4 μg/ml for all isolates tested. Considering that 97.8% were susceptible to concentrations of ⩽4 μg/ml of lomefloxacin, this new quinolone offers potential in the therapy of a variety of systemic infections, especially in patients with multiply resistant pathogens. The MIC90 for Klebsiella spp. and Pseudomonas spp. was 4 μg/ml. The MIC90 was 4 μg/ml for S. aureus and 2 μg/ml for S. epidemidis and S. saprophyticus. The MIC90 of Streptococcus spp. were ⩽4 μg/ml for all isolates tested. Considering that 97.8% were susceptible to concentrations of ⩽4 μg/ml of lomefloxacin, this new quinolone offers potential in the therapy of a variety of systemic infections, especially in patients with multiply resistant pathogens. No hubo recaídas ni muertes relacionadas con la micosis. El resultado clínico y radiológico fue satisfactorio y solo quedaron hallazgos residuales como disnea y fibrosis. Las pruebas serológicas mostraron una disminución de la actividad de los anticuerpos en comparación con los resultados obtenidos al inicio de la terapia. Estos hallazgos sugieren que el itraconazol constituye un tratamiento adecuado para los pacientes con paracoccidioidomicosis.
