Methods: Retrospective cohort of 97 patients diagnosed with fungemia and with or without involvement of the posterior segment. Demographic, clinical and ophthalmological variables were identified to establish the risk of retinal seeding. Results: An incidence of ocular involvement of 22.68% was obtained and no clear risk factor was found for subsequent showings in patients with fungemia. A risk trend was only found in patients with diabetes with an OR: 2.85; CI 95%: (0.80-10.12) and history of HIV with an OR: 2.29 CI95%: (0.85-6.12). Conclusions: In this first cohort carried out in Colombia according to our search, findings were obtained that agree with those of other authors worldwide, where there is no evidence of a decrease in incidence compared with older studies and the absence of risk factors for the compromise of the posterior pole in patients with fungemia.KEY MESSAGES Systematic fundus evaluation by an ophthalmologist in patients with candidaemia is a recommended practice based on low-quality evidence.The identification of real risk factors for retinal compromise in fungemia would allow us to be more selective with the population to be evaluated.Fungemia generally occurs in critically ill patients, where access and availability of ophthalmology evaluation are a resource that is not always available. We aim to assess the reproducibility of a standardized chest radiograph reading protocol in contacts of patients with pulmonary TB under the 5 years of age. Methods: Descriptive, cross-sectional study with children under the age of five, household contacts of patients with confirmed pulmonary TB from Medellín, Bello and Itagüí (Colombia) between Jan-01-2015 and May-31-2016. Standardized reading protocol: two radiologists, blinded independent reading, use of template (Dr. Andronikou design) in case of disagreement a third reading was performed. Kappa coefficient for intra and inter observer agreement, and prevalence ratio were estimated of sociodemographic characteristics, TB exposure and interpretation of chest X-ray. Results: From 278 children, standardized reading found 255 (91.7%) normal X-rays, 10 (3.6%) consistent with TB, and 13 (4.7%) other alterations. Global agreement was 91.3% (Kappa = 0.51). Inter-observer agreement between readers 1-2 was 90.0% (Kappa = 0.59) and 1-3 93.2% (Kappa = 0.59). Intra-observer agreement for reader 1 was 95.5% (Kappa = 0.86), 2 84.0% (Kappa = 0.51), and 3 94.7% (Kappa = 0.68). Greater inter-observer disagreement was between readers 1-2 for soft tissue density suggestive of adenopathy (4.6%), airspace opacification (1.17%) and pleural effusion (0.58%); between readers 1-3 for soft tissue density suggestive of adenopathy (4.2%), opacification of airspace (2.5%) and cavities (0.8%). Conclusions: Chest radiographs are an affordable tool that contributes to the diagnosis of TB, so having a standardized reading protocol showed good agreement and improves the reproducibility of radiograph interpretation. Objetivo: caracterizar los recién nacidos diagnosticados con SD y sus madres en unidades de atención materno infantil de la ciudad de Medellín. Adicionalmente, se exploraron factores asociados al diagnóstico oportuno antenatal. Método: estudio observacional descriptivo de tipo transversal con intención analítica basado en registros médicos de recién nacidos diagnosticados con SD en siete instituciones de Medellín, Antioquia entre enero de 2015 y diciembre de 2019 teniendo en cuenta variables tanto maternas como neonatales. Se realizó un análisis univariado mediante frecuencias absolutas y relativas, luego se realizó un análisis bivariado teniendo en cuenta los desenlaces neonatales según el acceso al control prenatal y el momento en el cual se realizó el diagnóstico (prenatal o neonatal) y finalmente se realizó un análisis multivariado para diagnóstico neonatal tardío. Resultados: el 50,2% de las madres al momento del parto tenía 35 años o menos, de estas el 59,9% pertenecía al régimen contributivo y el 83,4% realizó 4 controles prenatales (CPN) o más, a pesar de esto, se encontró que sólo el 33,7% tenían diagnóstico prenatal de SD. La mayoría (91,4%) de los neonatos tuvo diagnóstico de cardiopatía congénita y la estancia hospitalaria prolongada estuvo en relación a morbilidad relacionadas. Conclusión: el SD es una de las cromosomopatías más común en nuestro medio, la cual se puede diagnósticar de forma temprana. Sin embargo en nuestro estudio se pudo evidenciar que pese a la alta cobertura de CPN la frecuencia del diagnóstico antenatal es menor, lo cual requiere que estos controles sean realizados por personal médico entrenado en pacientes con este tipo de patología. Esto finalmente se va a ver reflejado en una mejor aceptación por parte de la familia hacia la llegada de un hijo con esta condición y por supuesto, mejor acceso a servicios de salud especializados. The vast majority of radiation-induced cancers are papillary carcinomas (PTCs). These tumors typically have gene fusions in contrast to point mutations prevalent in sporadic PTCs. The aim of this study was to investigate the molecular profiles of PTC patients with workplace exposure to ionizing radiation. Methods: A retrospective review of 543 patients who underwent surgery with diagnosis of PTC was performed. A cohort of nine healthcare specialists previously exposed to radiation sources during their professional practice was selected and analyzed using the ThyroSeq mutation panel for point mutations and gene fusions associated with thyroid cancer. Results: The molecular analysis of surgical samples of PTCs was informative and revealed genetic alterations in five patients. BRAF V600E was found in four (67%) cases whereas RET/PTC1 fusion in one (17%) and one sample (17%) was wild type for point mutations and fusions. One sample completely failed molecular analysis while two others were negative for genes fusions but failed DNA analysis; these three samples were excluded. Conclusions: In this limited cohort of healthcare workers exposed to low dose of ionizing radiation at the workplace and developed PTC, the molecular profiling determined BRAF V600E point mutation as the most common event, arguing against the role of workplace radiation exposure in the etiology of these tumors. The online course included: didactic lectures, small-group clinical case discussions, and panel case discussions. The course was delivered using Zoom® platform and designed to address four core competencies: prevention, definition and diagnosis, antimicrobial therapy, and surgical treatment. An online questionnaire was created distributing 16 questions through six clinical scenarios. Participants were invited to answer the questionnaire before and after the course. Sixty of the 78 course participants answered the pre-course, and 42 the post-course assessment. Relative to before the course, the mean post-course assessment score rose significantly for prevention of FRI (4.1 before and 4.5 after; p = 0.014), definition and diagnosis (2.4 before and 3.4 after; p = 0.001), and surgical treatment (2.2 before and 2.8 after; p = 0.011). The final score encompassing all four core competencies also rose significantly (2.7 before and 3.3 after; p = 0.001). The online course on FRI was feasible and effective, significantly increasing course users’ knowledge of overall competency in managing FRI. The objective of this study was to describe the clinical characteristics and severity of SARS-CoV-2 infection in pediatric patients seen at a reference hospital in Colombia. Method: A descriptive, observational study in patients under the age of 18 years with a positive test for SARS-CoV-2 infection (RT-PCR or antigen) between April 2020 and March 2021. Multiple variables were studied, including demographic data, clinical characteristics, lab measurements, treatments administered, intensive care unit admission, and mortality. Results: A total of 361 patients were included of whom 196 (54%) were males. The median age was 3 years. Of all the patients, 65 (18%) were asymptomatic. The majority of patients had no comorbidities (n = 225, 76%). In those who were symptomatic (n = 296, 82%), the most frequent complaints were fever (n = 178, 60%), nasal congestion (n = 164, 55%) and cough (n = 149, 50%). Chest x-rays were normal in 73 patients (50%). When abnormalities were found, interstitial (29%) and alveolar (12%) patterns were the most prevalent. One hundred and fifty-seven children (53%) required general ward hospitalization, and 24 patients (8%) required pediatric intensive care admission. The global mortality was 0.8% (3 patients). Conclusions: The majority of cases were asymptomatic or mild. However, a significant percentage of patients required general ward admission, and some even required intensive care. The main symptom of COVID-19 infections in newborns was apnea. A second COVID-19 RT-PCR may be necessary to detect infections in critically ill patients with a high clinical suspicion of the disease if an initial test was negative. Objetivo: el presente artículo presenta una revisión de la literatura con el fin de actualizar los conceptos de esta patología en el personal de la salud que está en contacto con la población pediátrica y que es sometida a procedimientos con medios de contraste. Metodología: en esta revisión narrativa de la literatura, presentamos la definición, los factores de riesgo, el enfoque clínico y las medidas preventivas de la nefropatía inducida por contraste en pediatría. Resultados: se define que hay un deterioro en la función renal aguda después de la administración del medio de contraste en donde se excluyen otras posibles etiologías y se establece una verdadera relación causal con la sustancia. Los factores de riesgo son múltiples, sin embargo, factores estrictamente relacionados en los niños no han sido establecidos en su totalidad. El abordaje de los pacientes que van a ser sometidos a estudios con medios de contraste inicia desde una historia clínica, un examen físico y unas medidas de laboratorio que permiten evaluar el estado basal de cada paciente para instaurar medidas preventivas. Por su parte, las estrategias de prevención de esta condición son múltiples, sin embargo, no existen guías basadas en la evidencia acerca de esta condición en el paciente pediátrico. Limited data regarding MM treatment in Latin America is available, and access to novel agents for a substantial portion of the population is limited by their high costs. 1 . In this multicenter cohort study, data on 890 Colombian MM patients are reported with a median follow-up of 18 months (IQR: 7-42 months). Patients were classified by age group (≤ or > 65 years). Median age at diagnosis was 67 years (IQR: 59-75 years) and 47.1% of patients were women. 709 patients (79.6%) received Bortezomib-based schemes as part of the first line, with Cyclophosphamide-Bortezomib-Dexamethasone (CyBorD) being the most frequently used (301 patients; 33.8%). Only two hundred and fifty-two patients (28.3%) were consolidated with Autologous Stem Cell Transplantation (ASCT) in first-line therapy. ASCT consolidation and age were the main independent factors influencing outcomes; in the non-ASCT cohort, 5-year overall survival was 48.7% (CI 41.8-55.2) compared to 80.7% (CI 73-86.4) in ASCT patients. This data depicts the reality of MM in Colombia, which likely reflects other Latin American countries, where access barriers to diagnosis and treatment are echoed in advanced stage diagnosis and a low rate of transplants. These seem to negatively impact survival despite the availability of most novel drugs approved in most of the world for this disease. Thus, emphasizing the paradox that prevails in most of the region: availability without equitable access. This study provides an overview of diagnostic and treatment services for patients with PNES across Latin America. Methods: In 2017–2018, clinicians practicing in Latin America with responsibilities for patients with PNES were contacted to respond to a survey regarding the management of this disorder developed by the International League Against Epilepsy (ILAE) PNES Task Force. Results: Three hundred and sixty responses from 17 Latin American countries were analyzed. Most respondents were neurologists (81%) under 40 years of age (61%). Fifty-seven percent of professionals stated that they personally diagnose PNES, but only 33% stated that they provide follow-up, and only 20% that they recommend treatment. Many participants (54%) characterized themselves as either unfamiliar with the diagnosis or inexperienced in arranging treatment. Most respondents reported having access to brain magnetic resonance imaging (MRI; 88%) and routine electroencephalogram (EEG; 71%), 64% have the access to video-EEG longer than 8 h, and 54% of professionals performed video-EEG to confirm PNES diagnoses. Although cognitive–behavioral therapy was recognized as the treatment of choice (by 82% of respondents), there was little access to it (60%). In contrast, a high proportion of respondents reported using antidepressant (67%), antiseizure (57%), and antipsychotic medications (54%) as treatments for PNES. Significance: This study reveals several deficiencies in the diagnosis and treatment of patients with PNES in Latin America. The barriers are reinforced by lack of knowledge among the specialists and poor healthcare system support. There is inadequate access to prolonged video-EEG and psychotherapy. An inappropriate use of antiseizure medicines seems commonplace, and there are low follow-up rates by neurologists after the diagnosis. Multidisciplinary guidelines are required to improve the approach of patients with PNES. Although cognitive–behavioral therapy was recognized as the treatment of choice (by 82% of respondents), there was little access to it (60%). In contrast, a high proportion of respondents reported using antidepressant (67%), antiseizure (57%), and antipsychotic medications (54%) as treatments for PNES. Significance: This study reveals several deficiencies in the diagnosis and treatment of patients with PNES in Latin America. The barriers are reinforced by lack of knowledge among the specialists and poor healthcare system support. There is inadequate access to prolonged video-EEG and psychotherapy. An inappropriate use of antiseizure medicines seems commonplace, and there are low follow-up rates by neurologists after the diagnosis. Multidisciplinary guidelines are required to improve the approach of patients with PNES. Por lo anterior, se plantean en este artículo de reflexión, diferentes propuestas para el tratamiento de la diabetes mellitus en la unidad de cuidados intensivos, sin descartar la forma ambulatoria, en donde la telemedicina y otras tecnologías permitirán acortar la distancia y mantener las medidas de aislamiento preventivo. The etiology can be chemical or mechanical; however, so far, there are no reports in the literature of an association with severe stenosis of the vena cava, as it is described in the case reported here, where a patient presented hemolysis in two hemodialysis sessions, without initially being possible to find the cause; the only identifiable factor was that he had a dysfunctional tunneled jugular catheter, with a history of difficult vascular access. The patient underwent interventional radiology, finding 99% stenosis of the vena cava, which prevented the passage of the contrast agent to the atrium. Angioplasty and catheter replacement were performed, with a resolution of the complication; the subsequent dialysis therapies were satisfactory. Objetivo: el objetivo de la presente revisión panorámica es evaluar la evidencia actual sobre el tratamiento citoreductor de la trombocitemia esencial disponible en Colombia. Métodos: se realizó una búsqueda estructurada de literatura respecto a la efectividad y seguridad de los tratamientos citoreductores en el paciente con trombocitemia esencial. A partir de la pregunta de investigación con estructura PICO, se definieron los criterios de inclusión: pacientes con trombocitemia esencial, en manejo con Anagrelida y/o Hidroxiurea. Se incluyeron estudios de revisiones sistemáticas, metanálisis, ensayos clínicos aleatorizados. Resultados: se realizó una búsqueda en MEDLINE, EMBASE, LILACS, Cochrane Database of systematic Reviews, que arrojó 119 referencias de las cuales se incluyeron cuatro estudios, que cumplían con las características de la población, intervención y comparador. Conclusiones: la terapia citorreductora es fundamental para el manejo de la trombocitemia esencial. La evidencia sugiere que tanto Hidroxiurea como Anangrelida son agentes que brindan una protección similar contra eventos trombo-hemorrágicos clínicamente significativos. Introduction: Salivary cortisol is an ultrafiltrate of plasma cortisol and reflects biologically active levels, this follows the circadian variation of serum cortisol, finding the highest levels in the morning and lower levels at midnight. Methods: A descriptive observational study was carried out in which healthy schoolchildren and adolescents between 6 and 18 years of age were included. Two samples of saliva were collected in each child, the first at 8:00 a.m. and the second at 23:00 a.m. on the same day. The samples were processed by enzymatic immunoassay (EIA-ELISA) in Abad laboratory of the city of Medellín. The statistical method of Kolmogorov-Smirnov and Shapiro-Wilk was used to determine normality. For the presentation of results, the measures of central tendency and dispersion were used according to the normality of the variable. Results: Data from 31 individuals were analyzed. 51.6 % of the population were men. The average age was 11.26 years (± 3.4) and the average BMI was 18.1 kg/m2 (± 2.8). With respect to cortisol levels, the cortisol level at 08:00 a.m. a median of 4.39 ng/ml was obtained (RIQ 2.76-6.98); and for the cortisol level at 11:00 p.m., a median of 0.51 ng/ml (RIQ 0.41-0.84) was obtained. There were no significant differences by sex. Conclusion: The average salivary cortisol values in the group of healthy schoolchildren and adolescents in the city of Medellín are within the reference values reported in international studies, so it is recommended for the diagnosis and monitoring of hyper / hypocortisolism. Results: The α/θ in all regions was lower in PD than in HC, with larger effect sizes in the posterior regions. Right parietal, and right and left occipital α/θ had significant positive correlations with performance in Judgement of Line Orientation Test (JLOT) in PD. Adjusted moderation analysis indicated that right, but not left, occipital α/θ influenced the JLOT performance related to PD. Conclusions: Reduction of the occipital α/θ, in particular on the right side, was associated with visuospatial performance impairment in PD. Significance: Visuospatial impairment in PD, which is highly correlated with the subsequent development of dementia, is reflected in α/θ in the right posterior regions. The most prevalent neoplasms were breast (132 [17,77%]), colorectal (92 12,34%), and prostate (81 [10,9%]). 220 (29,6%) patients were asymptomatic and 96 (26,3%) died. In the bivariate descriptive analysis, higher mortality in patients > 70 years, lung cancer, ≥2 comorbidities, former smokers, receiving antibiotics, corticosteroids, and anticoagulants, residents of rural areas, low socioeconomic status, and increased acute phase reactants. In the logistic regression analysis, higher mortality with ECOG 3 OR 28,67 (8,2-99,6), ECOG 4 OR 20,89 (3,36-129,7), two complications from COVID-19 OR 5,3 (1,50-18,1), and cancer in progression OR 02,08 (1,01-4,27). In the Cox regression analysis, the statistically significant HRs were metastatic disease HR 1,58 (1,16 - 2,16), cancer in progression HR 1,08 (1,24 - 2,61) vs partial response HR 0,31 (0,11 - 0,88), use of steroids HR 1,44 (1,01 - 2,06) and antibiotics HR 2,11 (1,47 - 2,95). Interpretations: In our study, cancer patients have higher mortality due to COVID-19 infection if they have active cancer, metastatic or progressive cancer, ECOG status >2 and low socioeconomic status. Implications for practice: Our findings raise the need to carefully evaluate patients with metastatic cancer, in progression, and with impaired ECOG to define the relevance of cancer treatment during the pandemic, consider the risk/benefit of the interventions, and establish clear and complete communication with the patients and their families about the risk of complications. There is also the importance of offering additional support to patients with low income and residence in rural areas so that they can have more support during cancer treatment. Se efectuó una búsqueda de la literatura de evidencia médica en diferentes buscadores como Science Direct y PubMed, usando las palabras claves “thromboprophylaxis”, “anticoagulation”, “thrombosis”, “anticoagulant”, “COVID-19”, “SARS-CoV-2”, “coronavirus”. Posteriormente se escribieron las recomendaciones generales referentes al tema. Without preoperative imaging, vascular anomalies such as having no sizable perforator are sometimes encountered during dissection. We propose a technique, based on a modified version of the traditional myocutaneous ALT to allow harvest of the flap based on non-sizable perforators. This technique can also enable splitting of a flap when only one sizable perforator is present. Methods: A retrospective review was done of patients who received reconstruction with free ALT flap from 2013 to 2019 by the senior author HSS and included all flaps in which non-sizable perforators were harvested. Data collected for analysis included patient demographics, flap size, defect location, inset type and flap survival. Surgical Technique: Despite detachment of the majority of skin paddle from the muscle, the flap is harvested with a sleeve of areolar tissue containing preferably more than one non-sizable perforators attached to a small muscular segment of the vastus lateralis containing the pedicle. Results: A total of 349 ALT flaps were performed during the review period by senior author HSS, and 25 flaps were harvested with non-sizable perforator, 10 of which were to enable a split. There were no total losses and 6 partial losses; 2 were amenable to direct closure after debridement, 1 required skin graft, and 3 required a new flap for wound coverage. Incorporating more than one non-sizable perforator increases the reliability of the flap. This technique should be used with caution in patients with multiple underlying comorbidities and when a flow-through flap is required. We were able to achieve primary closure of all donor sites. Conclusions: It is possible to harvest the anterolateral thigh flap without sizable perforators by conversion to a modified version of myocutaneous flap. In well selected patients, using our technique, several non-sizable perforators can reliably perfuse an ALT without the need to use an alternative donor site. This maximizes the number of harvestable ALTs and also increases the reconstructive potential by splitting previously “un-splitable” flaps. Without preoperative imaging, vascular anomalies such as having no sizable perforator are sometimes encountered during dissection. We propose a technique, based on a modified version of the traditional myocutaneous ALT to allow harvest of the flap based on non-sizable perforators. This technique can also enable splitting of a flap when only one sizable perforator is present. Methods: A retrospective review was done of patients who received reconstruction with free ALT flap from 2013 to 2019 by the senior author HSS and included all flaps in which non-sizable perforators were harvested. Data collected for analysis included patient demographics, flap size, defect location, inset type and flap survival. Surgical Technique: Despite detachment of the majority of skin paddle from the muscle, the flap is harvested with a sleeve of areolar tissue containing preferably more than one non-sizable perforators attached to a small muscular segment of the vastus lateralis containing the pedicle. Results: A total of 349 ALT flaps were performed during the review period by senior author HSS, and 25 flaps were harvested with non-sizable perforator, 10 of which were to enable a split. There were no total losses and 6 partial losses; 2 were amenable to direct closure after debridement, 1 required skin graft, and 3 required a new flap for wound coverage. Incorporating more than one non-sizable perforator increases the reliability of the flap.
