The systematic use of echocardiography in SAB is controversial. We aimed to validate VIRSTA and PREDICT scores for predicting the risk of IE in Colombian patients with SAB and, consequently, to determine the need for echocardiography. Methodology: Cohort of patients hospitalized with SAB in two high complexity institutions in Medellin, Colombia, between 2012-2018. The diagnosis of IE was established based on the modified Duke criteria. The VIRSTA and PREDICT scores were calculated from the clinical records, and their operational performance was calculated. Results: The final analysis included 922 patients, 62 (6.7%) of whom were diagnosed with IE. The frequency of IE in patients with a negative VIRSTA scale was 0.44% (2/454). The frequency of IE in patients with a negative PREDICT scale on day 5 was 4.8% (30/622). The sensitivity and negative predictive value (NPV) of the VIRSTA scale was 96.7% and 99.5%, respectively. For the PREDICT scale on day 5, the sensitivity and NPV were 51.6% and 95.1%, respectively. The discrimination, given by the area under the ROC curve (AUC), was 0.86 for VIRSTA and 0.64 for PREDICT. Conclusions: In patients with negative VIRSTA, screening echocardiography may be unnecessary due to the low frequency of IE. In PREDICT-negative patients, despite the low frequency of IE, it is not safe to omit echocardiography. The extensive functional overlap of these proteins results in shared phenotypical features, which complicate the assessment of the clinical diagnosis. Additionally, re-evaluation of individuals at a later age occasionally reveals that the initial phenotype has evolved toward clinical features more reminiscent of a developmental disorder different from the one that was initially diagnosed. For this reason, variants in ANKRD11 can be ascribed to a broader class of disorders that fall within the category of the so-called chromatinopathies. In this work, we report on the clinical characterization of 23 individuals with variants in ANKRD11. The subjects present primarily with developmental delay, intellectual disability and dysmorphic features, and all but two received an initial clinical diagnosis of either KBG syndrome or CdLS. The number and the severity of the clinical signs are overlapping but variable and result in a broad spectrum of phenotypes, which could be partially accounted for by the presence of additional molecular diagnoses and distinct pathogenic mechanisms. Materiales y métodos: se analizaron de forma retrospectiva datos de pacientes adultos con EII del Hospital Pablo Tobón Uribe en Medellín, Colombia, que consultaron desde 2001 hasta febrero de 2019. Se emplearon frecuencias absolutas y relativas. Para comparar dos proporciones se utilizó la prueba de chi cuadrado de independencia y se estimó el Odds ratio (OR). Resultados: se documentaron 759 pacientes con EII, de los cuales 544 (71,6 %) presentaron colitis ulcerativa (CU); 200 (26,3 %), enfermedad de Crohn (EC), y 15, EII no clasificable (1,9 %). En total, 185 (24,4 %) pacientes con EII presentaron anemia, que es menos frecuente en CU que en EC (22,2 % frente a 32,5 %, respectivamente; OR: 0,684; IC: 0,456-0,96; p = 0,03). Los pacientes con CU extensa (54,1 %) tenían más anemia que aquellos con CU no extensa (46,3 %) (OR: 4,4; IC: 2,6-7,4; p = 0,001); lo mismo se observó al comparar la CU grave (66,1 %) con la no grave (32,3 %) (OR: 4,95; IC: 2,87-8,51; p = 0,000). En la EC, los pacientes con comportamiento no inflamatorio (B2, B3: 73,9 %) presentaron más anemia en comparación con aquellos con comportamiento inflamatorio (B1: 26,2 %) (OR: 0,35; IC: 0,18-0,67; p = 0,000). 44,3 % de los pacientes no recibió tratamiento, 19,5 % recibió hierro oral, 20,0 % recibió hierro intravenoso y 16,2 % fue transfundido. Conclusiones: la anemia es una complicación frecuente de la EII en nuestro medio (24,4 %). A pesar de las guías internacionales, el tratamiento es subóptimo en nuestro medio. The usual therapeutic options are the injection of cyanoacrylate, the insertion of coils or both. The Hydrocoil™, are used in neurovascular interventionism, allowing a rapid occlusion of the vessel forming a mesh that favours the local formation of thrombus and the development of a neointima on the gel cover. We consider the EUS-guided hydrocoil insertion in gastric varices, without using cyanoacrylate. Objective: The aim of this study is to evaluate the safety and effectivity of the application of USE-guided hydrocoils in patients with gastric varices haemorrhage with TIPS contraindication. Material and methods: Retrospective series of cases including four patients with TIPS contraindication after evaluation form an interventional radiologist. We used linear echoendoscopes, fluoroscopy, 19G needles and hydrocoils (AZUR, Terumo)TM, ProgreatTM 3Fr microcatheters. The procedures were advised by an expert interventional radiologist. The varix obliteration is confirmed by endoscopic ultrasound. Results: Technical and clinical success was reached in all 4 patients included. There were no adverse effects related to the procedure or endoscopic equipment damage. Conclusions: The application of USE-guided Hydrocoils can be a safe and effective method in gastric varices bleeding in patients not candidates for TIPS. It could also ensure a complete obliteration of the vascular lumen and thus dispense with the use of cyanoacrylate. Longer studies are needed to corroborate these preliminary results. Tipo de documento : documento electrónico Autores : Gabriel Alonso Mosquera Klinger, Fecha de publicación : 2021 Títulos uniformes : Universitas Médica Idioma : Español (spa) Palabras clave : Enfermedad de Crohn tuberculosis inmunosupresión diagnostico diferencial Resumen : La Tuberculosis intestinal (TBI) y la enfermedad de Crohn (EC) son trastornos granulomatosos crónicos que podrían tener un compromiso transmural y afectar cualquier parte del tracto digestivo; en algunos casos estas entidades pueden tener una presentación similar desde el punto de vista clínico, radiológico, endoscópico e incluso histológico. presento 2 casos de pacientes con diagnóstico presuntivo inicial de EC con compromiso ileocolónico basados en los datos clínicos, hallazgos endoscópicos, serológicos, histológicos, pero con una respuesta clínica pobre a las terapias convencionales usadas en EC incluyendo esteroides; en ambos casos se descartaron inicialmente infecciones sistémicas y se iniciaron terapias inmunosupresoras con azatioprina y con anticuerpos contra factor de necrosis tumoral alfa (AntiTNFa) con lo cual desarrollaron formas diseminadas de tuberculosis, sin mejoría de las manifestaciones gastrointestinales; pero si tuvieron respuesta satisfactoria desde el punto de vista infeccioso y digestivo con los antituberculosos. En estos casos el diagnóstico es TBI mimetizando a la EC. Palabras clave : Enfermedad de Crohn tuberculosis inmunosupresión diagnostico diferencial Resumen : La Tuberculosis intestinal (TBI) y la enfermedad de Crohn (EC) son trastornos granulomatosos crónicos que podrían tener un compromiso transmural y afectar cualquier parte del tracto digestivo; en algunos casos estas entidades pueden tener una presentación similar desde el punto de vista clínico, radiológico, endoscópico e incluso histológico. presento 2 casos de pacientes con diagnóstico presuntivo inicial de EC con compromiso ileocolónico basados en los datos clínicos, hallazgos endoscópicos, serológicos, histológicos, pero con una respuesta clínica pobre a las terapias convencionales usadas en EC incluyendo esteroides; en ambos casos se descartaron inicialmente infecciones sistémicas y se iniciaron terapias inmunosupresoras con azatioprina y con anticuerpos contra factor de necrosis tumoral alfa (AntiTNFa) con lo cual desarrollaron formas diseminadas de tuberculosis, sin mejoría de las manifestaciones gastrointestinales; pero si tuvieron respuesta satisfactoria desde el punto de vista infeccioso y digestivo con los antituberculosos. En estos casos el diagnóstico es TBI mimetizando a la EC. We recommend negative pressure dressing as the gold standard technique for delayed cavity closure, associated with higher wall closure success rates and lower complication and mortality rates. This data are preliminary and need further independent studies to better assess the role of sex. The yield of vEEG examinations can be increased through suggestive seizure manipulation (SSM) (ie, activation/provocation/cessation procedures), but its use has raised ethical concerns. In preparation for guidelines on the investigation of patients with PNES, the ILAE PNES Task Force carried out an international survey to investigate practices of and opinions about SSM. An online questionnaire was developed by the ILAE PNES Task Force. Questions were asked at clinical unit or individual respondent level. All ILAE chapters were encouraged to send questionnaires to their members. The survey was open from July 1, 2019, to August 31, 2019. A total of 487 clinicians from 411 units across 94 countries responded. Some form of SSM was used in 296/411 units (72.0%). Over 90% reported the use of verbal suggestion, over 80% the use of activation procedures also capable of eliciting epileptic activity (hyperventilation or photic stimulation). Only 26.3% of units used techniques specifically intended to provoke PNES (eg, saline injection). Fewer than 10% of units had established protocols for SSM, only 20% of units required written patient consent, in 12.2% of units patients received explicitly false information to provoke seizures. Clinicians using SSM tended to perceive no ethical problems, whereas those not using SSM were likely to have ethical concerns about these methods. We conclude that the use of invasive nocebo techniques intended to provoke PNES in diagnostic settings has declined, but SSM is commonly combined with activation procedures also capable of eliciting epileptic activity. While research suggests that openness about the use of PNES-specific nocebo techniques does not reduce diagnostic yield, very few units have suggestion protocols or seek patient consent. This could be addressed through establishing consensus guidance for the practice of SSM.
